Copayment Policy: All copayments are due at the time of service as per your insurance policy. Our office staff will inform you of the copayment amount based on your plan. If you have questions about your copay or payment policies, please reach out to us before your appointment.

CareCredit Partnership
Foundation Healthcare partners with CareCredit to provide flexible financing options for healthcare expenses. Patients interested in applying for CareCredit can apply directly on the CareCredit website or contact our office for assistance.
In-House Payment Plan
We also offer an in-house payment plan allowing patients to split their payments into two installments. To qualify for this plan, a card must be kept on file, and patients are required to commit to a three-month membership if choosing this payment option. Please contact our office for more details and to set up this plan.
